REQUIRED FIELD

The Survey Template determines the looks of the survey. Select the template package you wish to use. A sub-window beneath this select box shows a preview of what your survey will look like with the template package selected in the Survey Template field.

With the certain plans you may be allowed to create your own template package. Any custom template packages you have created will also be listed in the Survey Template select box.
