You've already got two questions added. Now we're going to add a new page to collect the user's contact details. Once we've done that, we'll preview the whole survey!

[edit] Take a Look at Your Survey
Before we add a new page, let's return to our survey overview and look at what we've got. To do this, click on your survey's name in the top navigation bar. If you named it My First Survey like I did, it will look like this.
When you look at your survey, you should see three items listed under Questions and Content: an untitled page and your two questions. Pay attention to the Type column (highlighted in red below). You'll notice that the page is at the top and the two questions are below. Any questions listed directly below a page will be displayed on that page.
[edit] Add a New Page
To create a second page in our survey, just click the Add Page link directly above the content list. It will automatically be placed below the questions we've already added. On the page configuration screen, just fill out the Page Title (Step 1). We'll call this page Personal Details since we'll be collecting the user's information. Once that's done, click the Add Page button (Step 2).

Now we can add our questions to this new page by clicking the Add Question link.
Let's ask the user for his name, email address, phone number and postal address. In order to do this, we'll need to use the Text Field List question type, so that we can ask for multiple text lines at once.
First, fill out the Question Text so that it says "Contact Details" (Step 1). This will be displayed to the user above the different fields.
Next, you need to fill out the different text fields. Let's start with the user's name. Make sure the Text Field Type is set to "Text Field (Single Line)" (Step 2). This will put a single line text field for the user to fill in. Then set the Text Field Label to "Name", so that the user knows what information to put in this field (Step 3).
Let's require this field to be filled out, so that the user can't submit the survey without providing their name. To do this, make sure the checkbox next to Response Required is checked (Step 4). When you've done all of these things, go ahead and click the Add Text Field button (Step 5).
Now repeat these steps for the user's email address and phone number. Some users might not like to give their phone number over the internet, so skip Step 4 when you're setting up this text field. You don't want to make this response required.
You'll repeat the steps again for the user's postal address, but you need to make one adjustment to Step 2. Instead of setting the Text Field Type to "Text Field (Single Line)", you'll want to set it to "Text Area (Multiple Lines)". Again, you might not want to make this response required, so skip Step 4.
When you're done, you should have the list of text fields displayed like this.
Go ahead and click the Add Question button at the bottom of the page.
If you set up the single and multiple text line fields right, the question preview should look like this.

Now go back to your survey by clicking the survey's name in the top navigation panel.
Look over your survey details. If you used all the names and text recommended in this tutorial, your details should match the image below. When you're done, click the Preview button in the Survey and Appearance section. This will open your survey in a new window so that you can check to see if everything is working properly.
