The Survey Template determines the looks of the survey. Select the template package you wish to use. A sub-window beneath this select box shows a preview of what your survey will look like with the template package selected in the Survey Template field.

With the advanced plan you can create your own template package via the account manager. The custom created template packages will also be listed in the Survey Template select box. Enabling full control of exactly what your survey will look like.
